If any emergency were to arise with your pet and we were unable to contact you, please provide information of someone who could act on your behalf if needed (friend or family member).

Please enter the information on your pet that will be staying with us.

We require your pet to be up to date on their vaccinations, including Bordetella (Dogs only). Please provide the date of their last vaccinations. You may may also have your Veterinarian e-mail a copy or you may bring in a copy of their vaccinations upon arrival.

Please enter all feeding and medication information (if your pet takes medications). We will be sure to cater to their needs and feed them as if they were at home.

At Solid Gold we offer many exercise opportunities you may select for your pets. You can customize any schedule you think they would like. If you are not sure right now, we will always confirm upon your arrival!
